Qualifications

Responsibilities

• Assess and treat patients for physical injury or illness and cognitive, psychosocial, or developmental needs.
• Plan and educate patients; implement exercises and modalities for rehabilitation.
• Coordinate care with facilities when travel between locations is required.
• Coordinate with care teams and other providers; may travel between facilities.
