Position Overview

The Occupational Therapist is a vital member of the interdisciplinary team responsible for developing, administering, and implementing therapeutic services designed to help patients regain their optimal level of function in meaningful daily activities, including self-care, leisure, and work. The Occupational Therapist works under the supervision of the Home Health and Hospice Manager.

Essential Job Functions

• Evaluate patients' functional status and implement the physician's plan of care.
• Complete assessments/reassessments and make decisions based on objective tests and measurements.
• Collaborate with interdisciplinary team members to ensure the coordination of the plan of care.
• Provide education and direction to patients and coworkers as appropriate.
• Work independently as well as cooperatively with team members.
• Recognize and protect patient rights while conveying empathy and compassion to those in distress.
• Maintain accurate and concise documentation of all communications, coordination, and treatment activities.
• Adapt to emergency situations and remain alert to changes in patients' conditions.
• Support the organization's mission, vision, values, policies, and procedures.
• Participate in required education relevant to the position.
• Perform other related duties as assigned.

Qualifications

Education

• Required: Graduate of an occupational therapy curriculum accredited by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association; eligible for the National Registration Examination.
• Desired: Evidence of continuing education appropriate for skill level and area of practice.

Licenses/Certifications/Registrations

• Required: Licensed by the State of Oregon as an Occupational Therapist.
• Preferred: Registration as an Occupational Therapist with the American Occupational Therapy Association.
• CRASE training will be required if travel assignment extends beyond a six-month period.

Holiday coverage is required.
