Job summary

Apex Home Health, Hospice & Home Care is seeking a travel Occupational Therapist (OT) to evaluate, develop, and implement plans of care for patients with disabilities, injuries, or illnesses in an acute care setting. You will assess patient needs and coordinate care across settings while traveling between assignments in Illinois.

Responsibilities include reviewing histories, creating treatment plans, educating families and employers, and documenting progress to ensure compliance with guidelines.

Qualifications

• Minimum of 6 months experience (new grads encouraged).
• NBCOT-OTR certification.
• State licensure required (licensure may vary by state).
• BLS certification required.
• Strong knowledge of anatomy, physiology, pathology, and medical terminology.
• Ability to stand, walk, bend, and stoop comfortably.
