Job Description & Requirements

Roles And Responsibilities

• Perform comprehensive head-to-toe assessments each shift.
• Monitor and document vital signs, pain levels, intake/output, and clinical changes.
• Safely administer medications via oral, IV, IM, subcutaneous, and other routes.
• Monitor for side effects, adverse reactions, and therapeutic effects.
• Provide wound care, catheter care, ostomy care, and NG/PEG tube management.
• Manage IV fluids, drains, and wound vacs.
• Educate patients and families about medications, diagnoses, follow-up care, and self-care.
• Collaborate on discharge planning to ensure safe transitions to home or rehab.
• Accurately chart assessments, interventions, care plans, and patient outcomes in the EHR.
• Maintain legal and ethical standards in documentation.
