Job Description & Requirements

Unit Notes
  • Start Date: 6/17
  • Certifications: BLS
  • Facility Type: 1 unit; short term rehab, LTC, SNF, no locked memory. 28 beds, private rooms.
  • Typical census: 23-28 beds
  • EMR: Meditech, switching to a new system in June 2024
  • Staffing Ratios: RN: 8-hour shifts; coverage for 1 shift/day; ratio 1:23-28
  • Required Certifications: BLS; No IV certification needed for LPNs
  • Common diagnoses: Dementia, COPD, CHF, DM, HTN, among others
  • Job Duties: initial assessments, medication administration, overseeing CNAs, skin and pain assessments, admissions, wound care, pressure ulcer prevention, family communication
  • Lifts: Mechanical total lift system
  • Experience: Accepts first-time travelers
  • Team Meetings: Not required for RN in inpatient rehab
  • Manager: Theresa (available for calls); Charge RN or LPN
  • Physician coverage: 24/7 on hospital side
  • Interdisciplinary Team: PT, OT; daily meetings
  • Pharmacy: On-site with Bubble Packs
  • Shifts & Scheduling: Day (6:00-14:30), Evening (14:15-22:45), Night (22:30-07:00); may stay 4 hours past shift for overtime; possible shift rotation
  • Weekend & Holiday: EOW; may rotate into holiday schedule
  • Call/Standby: None
  • Floating: None
  • Scrub Color: Any
  • Orientation: 0.5 day general, 1-2 days with educator, 1-3 days with preceptor
