Required Qualifications:

• One year of recent experience as a Registered Nurse, including at least six months in a Home Health setting
• Proficient in OASIS documentation
• Valid RN license in compliance with state regulations
• BLS certification (AHA / ARC)

Preferred Qualifications:

• Home Health Nursing Certification (RN-BC)
• Other certifications and licenses may be required for this position

Summary:

A Home Health Registered Nurse (RN) provides direct nursing care to patients in their homes, ensuring comfort, stability, and effective management of medical conditions. Responsibilities include assessing patient conditions, developing care plans, administering treatments, and educating patients and caregivers on disease management.

Essential Work Functions:

• Assess patient conditions and develop individualized care plans
• Administer medications, treatments, and wound care, monitoring responses
• Educate patients and caregivers on medication management and self-care
• Monitor patient status, report concerns, and adjust care plans as needed
• Coordinate with healthcare professionals to ensure seamless care
• Document care interventions and progress accurately
• Ensure compliance with infection control and safety standards
• Support families and caregivers emotionally and practically
• Maintain communication with case managers and teams
• Complete charting and documentation efficiently
• Adhere to healthcare regulations and safety protocols
• Perform other duties within RN scope as assigned

Required Essential Skills:

• Proficiency in English (reading, writing, communication)
• Ability to follow instructions and use PC skills
• Physical ability to move and assist patients (50-100 lbs)
