The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes, following physician prescriptions and complying with applicable laws, regulations, and agency policies.

Responsibilities include coordinating the total plan of care with healthcare professionals, ensuring continuity of care through effective communication with physicians, patients, families, and community resources, all within a Point of Care setting.

The nurse performs initial and/or comprehensive evaluations, develops care plans within state guidelines, and submits accurate documentation in collaboration with physician orders.

Additional duties include conducting comprehensive assessments at designated times, developing appropriate care plans, and ensuring patients meet home health eligibility and medical necessity criteria as defined by the payer source.

Qualifications include at least 1 year of OASIS experience; homecare Homebase experience is preferred.
