Core Medical Group is seeking a travel nurse RN for a Home Health position in Geneva, New York.

Job Details

Additional Position: Pediatric RN - Home Care Oasis

Schedule: 32 hours/week, Friday - Sunday, 8am - 4:30pm. Training for first two weeks of contract, Monday - Friday, 8am - 4:30pm.

Requirements

• Certified Home Health Care Experience Required
• EPIC or Netsmart experience required

Responsibilities

• Develop care plans tailored to patients' needs in collaboration with families, physicians, and care teams.
• Complete OASIS assessments accurately reflecting patient needs.
• Maintain all required clinical documentation for compliance.
• Refer patients to appropriate healthcare systems and community resources.
• Communicate assessment outcomes to team members to ensure proper follow-up.
• Display professionalism, patience, and flexibility in interactions.
• Ensure problem resolution and maintain productivity standards.
• Adhere to Professional Home Visit Performance Standards.

Qualifications

• NY State Licensed RN
• One year of pediatric clinical experience preferred; home care experience advantageous
• Licenses & Certifications: RN, ACLS, BLS
• Skills: Strong assessment, communication, and interpersonal skills
• Technical Skills: Microsoft Office, electronic medical records, office equipment, internet/intranet
• Other: Valid driver’s license, auto insurance, ability to travel throughout the service area
