Responsibilities for the Case Management Assistant
  • Collaborate with Case Managers and Social Workers to assess patient needs and develop discharge plans.
  • Ensure referrals, authorizations, and transportation arrangements are completed.
  • Maintain accurate documentation in the electronic medical record (Epic).
  • Handle clerical tasks such as copying, faxing, scanning, and data entry.
  • Participate in daily team huddles and meetings to share knowledge and resolve issues.
Required Skills
  • Proficiency with Epic EHR.
  • Strong communication skills, both oral and written.
  • Ability to work independently.
  • Intermediate PC skills, including Word and data entry.
  • Excellent time management and problem-solving skills.
  • Must reside in PST timezone.
