• RN with 1 year minimum experience in case management.
• Certs required: BLS, driver's license, auto insurance.
• Experience with edema/CHF/COPD management and wound care.

Responsibilities

• Case manage 30 patients with RN:patient visits ratio.
• Perform cardiopulmonary and wound assessments; manage IVs and catheters.
• Provide patient education on diabetes, ostomy care, and meds.
