Job summary

Optum Home & Community Care, part of the Optum family, is hiring HouseCalls Advanced Practice Clinicians (Nurse Practitioners or Physician Assistants) to perform in-home health assessments across Idaho. This field-based role emphasizes education, gap closure, and coordination with primary care providers.

Annual compensation includes base pay plus potential incentives; travel is statewide, with flexible scheduling and no on-call requirements.

Qualifications

• Active certification in Family, Adult, Geriatric, or Adult‑Gerontology Primary Care (ANCC or AANP) or be eligible for it.
• Active and unrestricted state license with ability to obtain by start date
• Active and unrestricted driver's license with reliable transportation for home visits
• Willingness to travel statewide ( Idaho ) ~85% of time; able to work across the state

Responsibilities

• Perform comprehensive in-home assessments and health evaluations.
• Document diagnoses and care plans with accuracy.
• Communicate findings to PCPs to address gaps in care.
• Recognize urgent situations and intervene per protocol.
• Educate patients on disease processes, medications and adherence.
• Address social determinants of health and provide referrals as needed
