Job Summary :

We are seeking a dedicated and experienced RN Case Manager to work in an acute care hospital setting. The ideal candidate will manage discharge planning and coordinate patient care transitions while collaborating with social workers, physicians, and interdisciplinary teams.

Key Responsibilities :

  • Develop, implement, and evaluate individualized discharge plans for hospitalized patients
  • Collaborate with physicians and interdisciplinary care teams to determine appropriate discharge destinations
  • Coordinate referrals for continued care, including SNFs, home care, shelters, and specialty services

Required Qualifications :
Active NY State RN License (Primary Source Verified)
Minimum 1 year of inpatient case management experience
AHA BLS Certification – Required
PRI & Screen Certification - Preferred.
