Job summary

Atlas Healthcare, LLC is seeking an experienced Nurse Practitioner (NP) to perform initial assessments and follow-up visits across multiple Connecticut skilled nursing and long-term care facilities. This traveling role offers flexible scheduling, strong clinical support, and a dedicated medical scribe to assist with charting.

The NP will travel to designated facilities throughout the month to develop care plans, review records, and perform medically necessary follow-ups.

Qualifications

• Active Nurse Practitioner license in Connecticut.
• PointClickCare (PCC) experience required.
• Experience in SNF, long-term care, geriatrics, or post-acute care preferred.
• Willingness to travel to multiple facilities.

Responsibilities

• Perform initial comprehensive patient assessments
• Conduct follow-up visits as clinically indicated
• Review facility records, nursing notes, and patient histories
• Develop, document, and update individualized care plans
• Collaborate with facility staff and care teams
• Work alongside an assigned scribe for real-time documentation
• Ensure all documentation meets regulatory and billing requirements
