SNF Healthcare seeks a part-time Nurse Practitioner (NP) to perform initial patient assessments and follow-up visits across multiple Connecticut facilities. A dedicated medical scribe will assist with charting to minimize documentation time.

The NP will travel to designated facilities throughout the month to establish care plans and conduct medically necessary follow-ups, ensuring regulatory compliance and high-quality patient care.

Formación

• Active Nurse Practitioner license in Connecticut.
• PointClickCare (PCC) experience – required.
• Experience in SNF, long-term care, geriatrics, or post-acute care preferred.
• Strong clinical judgment and assessment skills.
• Ability and willingness to travel to multiple facilities.
• Excellent communication and organizational skills.

Job Description

We are seeking an experienced Nurse Practitioner (NP) to perform initial patient assessments and follow-up visits across multiple skilled nursing and long-term care facilities throughout Connecticut. This role offers flexibility, strong clinical support, and minimal documentation burden with a dedicated medical scribe assigned to assist with charting.

The NP will travel to designated facilities over the course of the month to complete comprehensive assessments, establish care plans, and perform medically necessary follow-ups.

Key Responsibilities
