Job summary

Champion Care seeks an experienced Travel MDS Coordinator RN to support our skilled nursing and rehabilitation communities across Wisconsin. The role offers autonomy and meaningful impact across multiple facilities.

You will function as a trusted clinical resource, strengthening MDS accuracy, care planning, and reimbursement while working with facility and regional leadership. Travel is required throughout Wisconsin.

Qualifications

• Must hold a current Wisconsin RN license or be eligible to obtain one.
• Experience as an MDS Coordinator in skilled nursing or long-term care.
• Knowledge of MDS 3.0, RAI, PDPM, OBRA, CMS requirements.

Responsibilities

• Complete and coordinate accurate MDS assessments.
• Support Resident Assessment Instrument processes per CMS guidelines.
• Review clinical documentation for accuracy and reimbursement opportunities.
• Coordinate interdisciplinary care planning and reflect resident needs.
• Monitor assessment schedules and submission deadlines.
• Provide education to MDS coordinators and nursing teams.
• Assist with PDPM, case mix, and quality measures.
• Identify opportunities to strengthen documentation and outcomes.
• Serve as an MDS resource during vacancies and transitions.
• Travel regularly to Champion Care communities in Wisconsin.

Travel MDS Coordinator RN

Are you the MDS expert everyone turns to when an assessment is due, a care plan needs attention, or a reimbursement question comes up?

Champion Care is seeking an experienced Travel MDS Coordinator RN to support our skilled nursing and rehabilitation communities throughout Wisconsin. This is an exciting opportunity for an MDS professional who enjoys variety, autonomy, and the opportunity to make a meaningful impact across multiple facilities.

Instead of being limited to one building, you will serve as a trusted clinical and regulatory resource for our nursing teams throughout Wisconsin. You will help strengthen MDS accuracy, care planning, compliance, and reimbursement while working alongside facility and regional leadership.

What You Will Do

• Complete and coordinate accurate, timely Minimum Data Set assessments
• Support the Resident Assessment Instrument process in accordance with CMS, OBRA, and state requirements
• Review clinical documentation for accuracy, completeness, and reimbursement opportunities
• Coordinate interdisciplinary care planning and ensure resident needs are accurately reflected
• Monitor assessment schedules, required documentation, and submission deadlines
• Provide education and support to MDS Coordinators, nursing leaders, and interdisciplinary teams
• Assist facilities with PDPM, case mix, quality measures, and survey readiness
• Identify opportunities to strengthen documentation, processes, and resident outcomes
• Serve as an MDS resource during vacancies, transitions, and periods of increased need
• Travel regularly to Champion Care communities throughout Wisconsin

What We Are Looking For

• Current Registered Nurse license in Wisconsin or eligibility to obtain one
• Previous MDS Coordinator experience in skilled nursing or long-term care
• Strong working knowledge of MDS 3.0, RAI guidelines, PDPM, OBRA assessments, care planning, and CMS requirements
• Ability to manage assessment schedules and meet required deadlines
• Strong clinical judgment, organization, and attention to detail
• Confidence working independently and supporting multiple nursing teams
• Excellent communication, education, and problem-solving skills
• RAC-CT certification is preferred but not required
• Willingness and ability to travel throughout Wisconsin
