Job summary

Skies HC and Rehab Center in Albuquerque, NM is seeking an experienced Travel LPN for a contract assignment. You will provide compassionate nursing care to residents in a long-term care and rehabilitation setting while collaborating with the interdisciplinary healthcare team.

The shift is Nights (PA), 6:00 PM to 6:30 AM, with Sunday-Monday-Tuesday schedule and 36 guaranteed hours per week. Active LPN/LVN license and current BLS are required; compact license preferred.

Qualifications

• Active LPN/LVN license (Compact license preferred).
• Minimum 1 year of LTC/SNF or rehab experience preferred.
• Current BLS certification.

Responsibilities

• Deliver direct patient care per physician orders and facility policies.
• Administer medications and treatments safely and accurately.
• Monitor, document, and report changes in patient conditions.
• Support resident care plans and assist with daily living activities.
• Maintain accurate nursing documentation and charting.
• Collaborate with RNs, physicians, therapists, and support staff.
• Ensure compliance with all facility, state, and federal regulations.
