Job Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team in a Long-Term Care (LTC) facility. The ideal candidate will have experience in geriatric or long-term care settings and a strong commitment to delivering high-quality, person-centered nursing care.

Key Responsibilities

• Provide direct nursing care to residents in accordance with individualized care plans and physician instructions
• Administer medications and treatments accurately and safely
• Monitor and document residents’ health status and report changes to supervising RN or physician
• Assist residents with activities of daily living (ADLs) as needed
• Perform basic nursing procedures including wound care, catheter care, and vital signs monitoring
• Collaborate with the interdisciplinary team to update and revise care plans
• Supervise and guide Certified Nursing Assistants (CNAs) to ensure high-quality care
• Maintain accurate records using the facility’s electronic health record (EHR) system
• Adhere to infection control, safety, and facility policies
• Participate in staff meetings, training, and shift handovers
• Provide emotional support and communicate effectively with residents and families
  
  

Qualifications

• Active LPN license in [State] (or Compact State, if applicable)
• Minimum of 1 year of LPN experience (LTC experience preferred)
• Current BLS certification (ACLS/other certs as required)
• Excellent clinical, organizational, and communication skills
• Ability to work independently and as part of a healthcare team
• Must be able to work scheduled shifts, including weekends and holidays as needed
