Malone Healthcare - Nursing is seeking a LPN / LVN for a travel job in Cook, Minnesota.

Type of Facility: 1 unit; short term rehab, LTC, SNF, no locked memory.

28 beds, private rooms.

Typical census 23-28.

• EMR: Meditech will be switching to a new system in June 2024.
• Ratios & Staffing: LPN: 8 hour shifts; covering the other 16 hours of the day; Ratio 1:23-28.
• Required Certifications: BLS; No IV Certification needed for LPNs.
• Common diagnosis: Dementia, COPD, CHF, DM, HTN, and more.
• Job Duties: LPN: follow up assessments, med pass, charge oversees CNAs, Skin assessments, Pain assessments, admissions, wound care, pressure ulcer prevention/treatments, assisting other staff as needed; family involvement and communication.
• Lifts: mechanical total lift system.
• Experience: Accepts first time travels.
• Weekend Requirements: EOW
• Holiday Requirements: May be asked to go into the Holiday Rotation.
