Job summary

Sweetwater Care is seeking an RN/LPN supervisor to provide nursing care within the scope of practice in a Texas facility. The role emphasizes charting accuracy, incident reporting, and adherence to state and local guidelines.

Ideal candidates will have long-term care experience, PCC charting proficiency, and an active LVN/LPN license in Texas, plus BLS/CPR certification or willingness to obtain. Management background is a plus but not required.

Qualifications

• Long Term Care experience required.
• Point Click Care charting experience required.
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Nursing management experience preferred but not required.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including nutrition, infections, skin conditions, weight loss or other changes.
• Utilizes the Quality Life Manual for patient identification, placement, and monitoring of programs.
• Document and notify applicable departments of patient room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
