Job summary

Long Island Select Healthcare, Inc. is seeking a Licensed Practical Nurse (LPN) to support a multisite health network in Suffolk County, NY.

You will assist RNs and physicians, monitor patient status, administer medications, document progress, and educate patients and families about care plans and health maintenance. The role requires NYS LPN license, CPR certification, and ability to travel between sites (Central Islip, Hauppauge, Riverhead, Smithtown, Port Jefferson).

Qualifications

• Associate's degree (A. A.) or equivalent from two-year college or technical school and one to six months' related experience and/or training; or equivalent combination of education and experience.
• New York State licensure as a Licensed Practical Nurse. Certified in CPR.

Responsibilities

• Review daily schedule and ensure setup of medical charts each day.
• Daily thorough cleaning of rooms and between patient turnovers.
• Obtain vital signs at each appointment.
• Obtain height and weight measurements as needed.
• Collect blood samples via venipuncture and process for lab testing as required.
• Perform routine urine sampling and culture testing as required.
• Perform EKGs as ordered by supervising clinician.
• Assist physicians and NPs/PAs with direct patient care as needed.
• Assist in Gynecology clinic and prepare Pap slides for lab testing.
• Prepare and sterilize instruments and manage medical records.
• Obtain authorization for prescriptions and procedures as needed.
• Triage telephone calls and maintain lab specimen logs.
• Maintain office supplies and inventory; order medical and lab supplies.
• Instruct patients regarding procedures and self-care; disseminate PCMH information.
• Coordinate appointments; support general medical operation of the Health Center.
• Participate in Infection Control and safety initiatives.
