Provide high-quality long-term care and resident-centered care in a skilled nursing facility
Administer medications and treatments according to physician orders and facility protocols
Monitor, assess, and document resident conditions and progress
Collaborate with physicians, nurses, CNAs, and other interdisciplinary team members
Maintain accurate documentation in the facility's EHR
Follow infection control procedures and facility safety standards
Communicate changes in resident condition appropriately
Support residents' physical, emotional, and overall well-being
Maintain compliance with facility policies and state and federal regulations
Valid Wisconsin RN or LPN license
High school diploma or equivalent
Current CPR/BLS certification
Minimum of 1 year LTC/SNF experience
Strong clinical skills
Excellent communication and teamwork skills
Local or semi-local to Southwest Wisconsin
Ability to work PM shifts 2:30 PM - 11:00 PM
Flexible scheduling with 5-8 hour shifts
Every other weekend required
