Job summary

HealthForce Healthcare in Vermont seeks an experienced Travel Long Term Care LPN to provide person-centered care in skilled nursing and long-term care settings. You will perform routine assessments, manage chronic conditions, and coordinate care with an interdisciplinary team in a fast-paced environment.

The role requires a current LPN license (Vermont or compact), BLS/CPR, and 1+ year LTC/SNF experience. Travel nursing, flexible shifts, and housing stipends are available.

Qualifications

Graduate of an accredited practical nursing program.
Active LPN license in Vermont or a valid multi-state Compact license.
Current BLS/CPR certification (AHA preferred).
Minimum 1+ year experience in LTC/SNF.
Strong interpersonal and clinical communication skills; proficiency with EHR documentation.
Open and willing to work all shifts, including weekends and holidays.

Responsibilities

Administer scheduled medications, treatments, and monitor resident responses.
Monitor and record vital signs; report acute changes to RNs/physicians.
Perform wound care and routine clinical procedures per care plans.
Document thoroughly in the facility’s EHR system.
Collaborate with CNAs, therapists, physicians, and families to foster compassionate care.
Support residents with ADLs and rehabilitation goals, ensuring safety and infection control.

Responsibilities

Administer scheduled medications, treatments, and monitor resident responses over time
Monitor and record vital signs; assess and promptly report acute changes in resident status to supervising RNs and attending physicians
Perform wound care, dressing changes, and routine clinical procedures in accordance with individual care plans
Maintain clear, thorough, and timely documentation in the facility’s Electronic Health Record (EHR) system
Collaborate closely with CNAs, therapists, attending physicians, and resident families to foster a compassionate care environment
Support residents with activities of daily living (ADLs) and rehabilitation goals when necessary, ensuring safety and infection control protocols are strictly upheld

Qualifications & Requirements

Education: Graduate of an accredited practical nursing program
Licensure: Active, unencumbered LPN license in the State of Vermont (or valid multi-state Compact license)
Certifications: Current BLS / CPR certification (AHA preferred)
Experience: Minimum of 1+ years of recent experience in a long-term care (LTC) or skilled nursing facility (SNF)
