Provide compassionate, safe, and individualized nursing care to patients.
Administer medications and treatments within the LPN scope of practice.
Monitor patients’ conditions and promptly report changes to the appropriate healthcare provider.
Perform routine nursing assessments and document patient information accurately.
Assist patients with activities of daily living and provide comfort measures.
Perform wound care, treatments, and other nursing procedures as assigned.
Maintain accurate and timely clinical documentation.
Collaborate with RNs, physicians, CNAs, and other members of the healthcare team.
Educate patients and families regarding medications, treatments, and care plans.
Follow facility policies, infection-control standards, and patient-safety protocols.
Promote a respectful, compassionate, and patient-centered environment.
Active LPN/LVN license eligible to practice in New Hampshire.
Current BLS certification preferred/required per facility requirements.
Strong clinical assessment and communication skills.
Ability to work independently while functioning effectively as part of a multidisciplinary team.
Reliable, compassionate, and adaptable.
Comfortable working evening shifts from 3 PM-11 PM.
