Job summary

Cross Country Allied is seeking a Physical Therapist for a travel assignment in Fort Wayne, Indiana. The PT IP role involves evaluating patients, developing and adjusting treatment plans, and collaborating with the care team to facilitate recovery.

The position runs for 13 weeks with a 40-hour work week, typically Monday–Friday, 8:00 AM to 5:00 PM, and requires a current PT license and BLS certification.

Qualifications

• BLS certification required prior to first shift; valid state license.
• Minimum of 2 years of PT experience; LTACH experience highly desired.

Responsibilities

• Evaluate patients and develop/modify treatment plans.
• Collaborate with interdisciplinary teams to facilitate patient recovery.
• Document medical records accurately, including daily notes, goals, and discharge summaries.
• Maintain licensure and adhere to regulatory standards and ethics.
• Participate in student and professional development programs and provide supervision as appropriate.
