Qualifications

• Virginia State Pharmacist license required.
• Hospital experience of at least 2 years required.
• Experience managing complex patients such as trauma, burn, or critical care.
• Proficiency with Dose Edge IV, Pyxis, Meditech Vigilanz.
• Experience with Vancomycin AUC dosing, Aminoglycoside dosing, renal dosing, warfarin, enoxaparin, electrolyte protocols, heparin, and DOACs.

Responsibilities

• Verify and compound IV medications.
• Perform medication verification and medication management using advanced protocols.
• Collaborate with healthcare teams to ensure safe medication practices in a Level 1 Trauma Center and Stroke Center.

This role involves collaboration with healthcare teams to ensure safe medication practices and adherence to hospital policies.

Job Details

• Required Qualifications: Virginia State Pharmacist license; hospital experience of at least 2 years; skills in managing complex patients such as trauma, burn, or critical care; proficiency in Dose Edge IV Experience, Pyxis, Meditech Vigilanz; experience with Vancomycin AUC dosing, Aminoglycoside dosing, Renal dosing, warfarin, enoxaparin, electrolyte protocols, heparin, and DOACs.
• Schedule: Evenings 2:30 PM 7:00 AM; 36 hours per week.
• Additional Information: The Pharmacist will verify and compound IV medications, perform medication verification, and utilize advanced protocols for medication management.
