Job summary

CloudinfraIT Inc. seeks a Travel Hospice RN for a Moab, Utah assignment. The role covers comprehensive patient assessments, pain and symptom management, and end-of-life education, with 32 hours weekly over 8-hour day shifts on a 13-week contract.

You will hold a Utah RN license (or Compact), with a minimum of 3 years RN experience and BLS certification. Experience with Meditech HospiceSoft/Careficient is preferred; expect on-call and weekend coverage and collaboration with an interdisciplinary

Qualifications

• Active Utah RN License or Compact RN License
• Minimum 3 Years of RN Experience
• Hospice, Home Health, or Palliative Care Experience Preferred
• Current BLS Certification
• Valid Driver's License & Reliable Transportation
• Strong Assessment, Communication, and Care Coordination Skills

Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans
• Provide pain and symptom management for hospice patients
• Educate and support patients and families regarding end-of-life care
• Collaborate with physicians, social workers, chaplains, and interdisciplinary teams
• Maintain accurate and timely documentation
• Participate in on-call rotation and weekend coverage as needed
