Job summary

HonorVet Technologies is seeking a compassionate Hospice RN for a travel assignment in Basking Ridge, New Jersey. The role covers 21 weeks, 37.5 weekly hours, days shift, with EMR documentation and interdisciplinary collaboration.

Qualifications

Active New Jersey RN license or compact state license.
Minimum 2 years Home Hospice RN experience.
BLS certification required.
Associate Degree in Nursing or Diploma in Nursing with active RN license.

Responsibilities

Perform comprehensive patient assessments and develop individualized hospice care plans.
Manage a hospice caseload while coordinating care with the interdisciplinary team.
Provide compassionate patient and family education and support.
Complete accurate and timely EMR documentation.
Reconcile medications and ensure safe medication administration.
Participate in interdisciplinary team (IDT) meetings and quality improvement initiatives.
