Job summary

Fusion Medical Staffing seeks a Home Health RN for a 13-week travel assignment in Burnsville, MN. You will deliver direct in-home nursing care, develop care plans, administer treatments, and educate patients and caregivers.

The role requires RN license in good standing, 1 year of recent RN experience with at least 6 months in Home Health, and BLS certification. Travel compensation and comprehensive benefits are provided.

Qualifications

One year of recent RN experience including at least six months in Home Health.
Proficient in OASIS documentation.
Current RN license compliant with state regulations.
BLS certification (AHA/ARC).

Responsibilities

Assess patient conditions and develop individualized care plans.
Administer medications, treatments, and wound care; monitor responses.
Educate patients and caregivers on medications, disease progression, and self-care.
Coordinate with physicians and therapists for seamless transitions.
Document care and progress per regulatory standards.
Maintain infection control and safety in home care settings.
Support families with guidance and emotional support throughout care.
Communicate with case managers and interdisciplinary teams.
Complete charting and meet visit/productivity goals.
Perform other duties within scope of practice.
