The candidate should have at least 1 year of Medicare-certified Home Health experience, with HCHB EMR familiarity preferred. Compensation is pay range $2,400–$2,600 per week, reflecting travel benefits and eligibility for standard

Qualifications

• 1 year of Medicare certified Home Health experience required.
• HCHB EMR experience preferred.

Responsabilités

• Provide one-on-one care to patients in their homes according to the plan of care.
• Complete patient assessments and communicate progress to the care team.
• Support individualized treatment plans and document care per assignment guidelines.
• Collaborate with families and interdisciplinary providers to improve patient outcomes.
