Job summary

American Traveler, LLC is seeking a Home Health Physical Therapist with 2+ years of home health experience and proficiency in HCHB and Oasis charting. The role covers geriatric patients across White, Cumberland, and Van Buren Counties with a 50–75 mile radius.

Day shift, 5x8 hours from 08:00 to 17:00, with mileage reimbursement and no weekend/on-call requirements unless targets demand. Local candidates acceptable; travel required within territory.

Qualifications

• Minimum 2 years of home health experience.
• Active state PT license (no pending licenses).
• Current BLS certification.
• HCHB and Oasis charting experience required.

Responsibilities

• Cover geriatric patients in home health with start of care and follow-up visits.
• Maintain a caseload of 6-7 patients per day; 3 visits worth of points possible.
• Meet minimum productivity of 30 points per week.
• Day shift, 5x8 hours, 08:00–17:00.
• No weekend or on-call requirements unless needed to hit targets.
• Territory spans White, Cumberland, and Van Buren Counties, 50–75 miles radius.
• Mileage reimbursed at IRS standard rate.
• Charts with HCHB and Oasis; dress code scrubs or polo with scrub pants.
• Local candidates accepted; first-time travelers not considered.
