Supplemental Health Care is seeking a travel Home Health Physical Therapist to provide patient-centered care in Las Cruces, New Mexico. The role involves evaluating patients, developing plans of care, and coordinating with an interdisciplinary team.

The position is a 13-week travel assignment, with 40 hours per week, 8-hour days. You must have an active PT license in the assignment state and BLS; mileage and travel between patients are expected.

Qualifications

• Active, unrestricted PT license in the state of assignment.
• Current BLS certification (AHA).
• 1+ year of recent home health experience.
• Experience conducting PT evaluations and developing therapy plans.
• Familiar with Medicare-certified home health guidelines and OASIS-E documentation.
• Experience with EMRs such as Homecare Homebase, NetSmart, or Epic.
• Clear background check, driving record, and up-to-date health clearance (TB, immunizations).
• Reliable transportation with valid auto insurance and ability to travel between patient homes.

Responsabilités

• Perform initial and ongoing PT assessments and develop individualized plans of care.
• Provide skilled interventions for strengthening, endurance, pain management, and fall prevention.
• Conduct gait and balance evaluations, home safety assessments, and mobility training.
• Educate patients and caregivers on exercise programs, adaptive techniques, and safety strategies.
• Collaborate with RNs, OTs, SLPs, and physicians to ensure coordinated, interdisciplinary care.
• Document all care in compliance with Medicare, OASIS-E, and agency standards.
