Supplemental Health Care, Inc. is seeking a Home Health Physical Therapist for a travel assignment in Nutley, New Jersey. The role requires 40 hours per week, 5–6 visits daily, and a 13-week duration with a travel-friendly schedule.

The ideal candidate holds an active PT license in the assignment state, BLS, at least one year of home health experience, and familiarity with EMRs such as Homecare Homebase, NetSmart, or Epic. Travel between patient homes is required, with compensation paid hourly.

Qualifications

• Active, unrestricted Physical Therapist license in the state of assignment.
• Current BLS certification (AHA).
• At least 1 year of recent home health experience.
• Experience conducting PT evaluations and developing plans of care.
• Familiar with Medicare home health guidelines and OASIS-E.
• Experience with EMRs such as Homecare Homebase, NetSmart, or Epic.
• Clear background check, driving record, and health clearance.
• Reliable transportation and ability to travel between patient homes.

Responsibilities

• Perform initial and ongoing PT assessments and develop care plans.
• Provide interventions for strengthening, endurance, pain management, fall prevention.
• Conduct gait/balance evaluations, home safety assessments, mobility training.
• Educate patients and caregivers on exercise, adaptive techniques, safety.
• Collaborate with RNs, OTs, SLPs, and physicians for coordinated care.
• Document all care per Medicare, OASIS-E, and agency standards.
