Qualifications

• Active, unrestricted Physical Therapist license in the state of assignment.
• Current BLS certification (AHA).
• At least 1 year of recent home health experience.
• Experience conducting PT evaluations and developing plans of care.
• Familiar with Medicare home health guidelines and OASIS-E.
• Experience with EMRs such as Homecare Homebase, NetSmart, or Epic.
• Clear background check, driving record, and health clearance.
• Reliable transportation and ability to travel between patient homes.

Responsibilities

• Perform initial and ongoing PT assessments and develop care plans.
• Provide interventions for strengthening, endurance, pain management, fall prevention.
• Conduct gait/balance evaluations, home safety assessments, mobility training.
• Educate patients and caregivers on exercise, adaptive techniques, safety.
• Collaborate with RNs, OTs, SLPs, and physicians for coordinated care.
• Document all care per Medicare, OASIS-E, and agency standards.

• $2,122 - $2,351 per week*
• 26-week contract
• Paid by the hour - not by the visit
• 5–6 visits per day (45–90 minutes each, based on complexity)
• Travel time factored into productivity expectations
