Job summary
Ardor Health Solutions is seeking a Travel Physical Therapist for a 13-week Home Health assignment in Charlotte, North Carolina. This role offers autonomy in delivering one-on-one care to adult and geriatric patients in patients' homes, with a full-time 40-hour week and 8-hour daytime shifts.
You will evaluate, treat, and educate patients, collaborate with a care team, and ensure compliance with home health standards.

- Perform comprehensive physical therapy evaluations and reassessments.
- Develop individualized treatment plans based on physician orders and patient goals.
- Treat orthopedic, neurological, post-surgical, cardiopulmonary, and medically complex patients.
- Provide gait training, balance training, therapeutic exercise, transfer training, and fall prevention education.
- Educate patients and caregivers on home exercise programs, safety strategies, and equipment use.
- Collaborate with nurses, physicians, case managers, and interdisciplinary care teams.
- Complete timely documentation and maintain home health compliance standards.
