Job summary
Ardor Health Solutions is seeking a Travel Physical Therapist for a Home Health assignment in Orangeburg, South Carolina. This 12-week travel role offers independent, one-on-one patient care in a Home Health setting, with 36-hour weeks and 12-hour day shifts, in a relaxing Southern locale.
You will travel between patient homes, provide evaluations and treatment plans, and help patients regain mobility and independence while collaborating with a local interdisciplinary team.
Qualifications
- Active South Carolina Physical Therapist license or eligibility to obtain.
- Previous Home Health experience preferred.
- OASIS assessments experience preferred.
- Familiarity with Home Health EMR systems, including Homecare Homebase (HCHB), is a plus.
- Current BLS certification.
- Strong clinical reasoning, time-management, and communication skills.
- Ability to work independently and travel between patient homes.
Responsibilities
- Provide skilled physical therapy services to patients in their homes.
- Complete comprehensive evaluations and develop individualized treatment plans.
- Help patients improve strength, balance, mobility, safety, and functional independence.
- Provide gait training, transfer training, therapeutic exercise, and fall-prevention education.
- Assess home safety and recommend appropriate assistive devices.
- Educate patients and caregivers on home exercise programs and safe mobility techniques.
- Collaborate with physicians, nurses, and the interdisciplinary Home Health team.
- Complete accurate and timely clinical documentation.
- Perform required Home Health assessments according to agency guidelines.
