Pride Health is seeking a Travel/Home Health Physical Therapist to join our client’s Home Health team in Tallahassee, FL. The role runs for 14 weeks, delivering 40 hours per week from Monday to Friday, with 8-hour days.

Mileage reimbursement is provided for travel between patient visits within Leon, Wakulla, Madison, Jefferson, and Taylor counties. The PT will perform evaluations, develop Plans of Care, and administer evidence-based interventions to restore function and independence in the home

Qualifikationen

• Active Physical Therapist license required.
• Current CPR certification.
• Valid Driver's License with reliable transportation.
• Strong patient assessment and treatment planning skills.

Position Summary

• We are seeking a dedicated Physical Therapist (PT) to provide skilled home health therapy services throughout the Tallahassee service area.
• The PT will evaluate patients, develop individualized treatment plans, and deliver evidence-based interventions designed to restore function, improve mobility, and enhance overall quality of life in the home setting.

Key Responsibilities:

• Perform comprehensive physical therapy evaluations and determine patient eligibility for therapy services.
• Develop, implement, and revise individualized Plans of Care based on physician referrals, patient assessments, and clinical goals.
• Provide therapeutic interventions for orthopedic, neurological, and neuromuscular conditions.
• Improve patient mobility, strength, balance, endurance, safety, and functional independence.
• Educate patients and caregivers regarding therapeutic exercises, home safety, fall prevention, and adaptive equipment use.
• Complete accurate and timely clinical documentation, including required OASIS assessments and progress notes.
• Coordinate care with physicians, nurses, caregivers, and interdisciplinary healthcare team members.
• Ensure compliance with Medicare, CMS, agency, and regulatory requirements.

Required Qualifications

• Active Physical Therapist (PT) License.
• Current CPR Certification.
• Valid Driver's License, Reliable Transportation, and Auto Insurance.
• Strong patient assessment and treatment planning skills.
