Responsibilities

• Perform initial therapy evaluations and establish plan of care.
• Carry out subsequent visits, monitor progress and adjust plan as needed.
• Supervise PTA and coordinate with care team.
• Maintain accurate documentation and compliance with regulations.

Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• PT licensure in Oklahoma
• Home health experience including HCHB

Assignment Details

• Schedule: Monday through Friday, 8:00 AM to 5:00 PM
• Occasional weekend need
• Desired territory includes Garfield, Grant, Alfalfa, Woods, Kingfisher, and Blaine counties
• Key duties include performing initial therapy evaluations, developing plan of care, completing subsequent visits as needed, and supervising PTA
