Supplemental Health Care is seeking a travel Home Health Physical Therapist for a travel job in Mayfield, Kentucky. The role involves providing compassionate, patient-centered care across home health visits with 40 hours per week and 8-hour shifts.

Candidates must hold an active PT license in the state, a current BLS certification, and at least 1 year of home health experience. The position includes initial assessments, developing plans of care, and collaboration with RNs and other clinicians.

• Active, unrestricted Physical Therapist license in the state of assignment.
• Current BLS certification (AHA).
• 1+ year of recent home health experience (within the last 2 years).
• Experience conducting PT evaluations and developing therapy plans.
• Familiar with Medicare-certified home health guidelines and OASIS-E documentation.
• Experience with EMRs such as Homecare Homebase, NetSmart, or Epic.
• Clear background check, driving record, and up-to-date health clearance (TB, immunizations).
• Reliable transportation with valid auto insurance and ability to travel between patient homes.

Home Health Physical Therapist Contract Details

• $2,044 - $2,225 per week*
• 26-week contract
• Paid by the hour - not by the visit
• 5–6 visits per day (45–90 minutes each, based on complexity)
• Travel time factored into productivity expectations
