Supplemental Health Care is seeking a travel Home Health Physical Therapist in Springfield, Ohio to provide patient-centered care across the region. The assignment runs for 13 weeks with 40 hours per week, primarily dayshift (8 hours) and travel between patient homes.

Ideal candidates hold an active PT license in Ohio, current BLS, and 1+ year of home health experience, familiar with Medicare/OASIS-E and able to work with EMR systems like Homecare Homebase, NetSmart, or Epic.

Qualifications

• Active Physical Therapist license in the state of assignment.
• Current BLS certification (AHA).
• 1+ year of recent home health experience.
• Experience conducting PT evaluations and developing therapy plans.
• Familiar with Medicare-certified home health guidelines and OASIS-E documentation.
• Experience with EMRs such as Homecare Homebase, NetSmart, or Epic.
• Clear background check, driving record, and up-to-date health clearance (TB, immunizations).
• Reliable transportation with valid auto insurance and ability to travel between patient homes.

Responsibilities

• Perform initial and ongoing PT assessments and develop individualized plans of care.
• Provide skilled interventions for strengthening, endurance, pain management, and fall prevention.
• Conduct gait and balance evaluations, home safety assessments, and mobility training.
• Educate patients and caregivers on exercise programs, adaptive techniques, and safety strategies.
• Collaborate with RNs, OTs, SLPs, and physicians to ensure coordinated, interdisciplinary care.
• Document all care in compliance with Medicare, OASIS-E, and agency standards.

• 1+ year of recent home health experience (within the last 2 years)
• Experience conducting PT evaluations and developing therapy plans

Home Health Physical Therapist Contract Details:

• $1,864 - $2,041 per week*
• 13-week contract
• Paid by the hour - not by the visit
• 5–6 visits per day (45–90 minutes each, based on complexity)
• Travel time factored into productivity expectations
