Job summary

Supplemental Health Care is seeking Home Health Physical Therapists to provide patient-centered care in Marquette, Michigan. This 13-week travel assignment requires 40 hours per week with 8-hour shifts, traveling between patient homes and performing PT evaluations and treatment.

You will use EMR systems like Homecare Homebase, NetSmart, or Epic, and collaborate with a multidisciplinary team to ensure compliant, evidence-based therapy plans and quality patient outcomes.

Qualifications

• Active, unrestricted PT license in the state of assignment.
• Current BLS certification (AHA).
• 1+ year of recent home health experience (within the last 2 years).
• Experience conducting PT evaluations and developing therapy plans.
• Familiar with Medicare-certified home health guidelines and OASIS-E documentation.
• Experience with EMRs such as Homecare Homebase, NetSmart, or Epic.
• Clear background check, driving record, and up-to-date health clearance (TB, immunizations).

Responsibilities

• Perform initial and ongoing PT assessments and develop individualized plans of care.
• Provide skilled interventions for strengthening, endurance, pain management, and fall prevention.
• Conduct gait and balance evaluations, home safety assessments, and mobility training.
• Educate patients and caregivers on exercise programs, adaptive techniques, and safety strategies.
• Collaborate with RNs, OTs, SLPs, and physicians to ensure coordinated, interdisciplinary care.
• Document all care in compliance with Medicare, OASIS-E, and agency standards.

• Experience conducting PT evaluations and developing therapy plans

• Reliable transportation with valid auto insurance and ability to travel between patient homes

Home Health Physical Therapist Contract Details:

• $2,030 - $2,215 per week*
• 13-week contract
• Paid by the hour - not by the visit
• 5-6 visits per day (45-90 minutes each, based on complexity)
• Travel time factored into productivity expectations
