Supplemental Health Care is seeking a travel Home Health Physical Therapist for a 13-week assignment in Richmond, Virginia. You will provide patient-centered PT services in home health settings, working 40 hours per week with 8-hour day shifts and travel between patient homes.

• 1+ year home health experience within last 2 years.
• Experience conducting PT evaluations and developing therapy plans.
• Familiar with Medicare-certified home health guidelines and OASIS-E documentation.
• Experience with EMR systems (Homecare Homebase, NetSmart, or Epic).
• Clear background check, driving record, and health clearance.

Responsibilities

• Perform initial and ongoing PT assessments and develop individualized plans of care.
• Provide interventions for strengthening, endurance, pain management, and fall prevention.
• Conduct gait and balance evaluations, home safety assessments, and mobility training.
• Educate patients/caregivers on exercise programs and safety strategies.
• Collaborate with RNs, OTs, SLPs, and physicians to ensure coordinated care.
• Document all care in compliance with Medicare, OASIS-E, and agency standards.

Job Description

Supplemental Health Care is hiring Home Health Physical Therapists (PTs) to provide compassionate, patient-centered care in Richmond, Virginia and the surrounding areas. With 40+ years of experience in healthcare staffing, SHC connects clinicians with top home health partners nationwide.

Qualifications

• Active, unrestricted Physical Therapist (PT) license in the state of assignment


• Current BLS certification (AHA)


• 1+ year of recent home health experience (within the last 2 years)


• Experience conducting PT evaluations and developing therapy plans


• Familiar with Medicare-certified home health guidelines and OASIS-E documentation


• Experience with EMRs such as Homecare Homebase, NetSmart, or Epic


• Clear background check, driving record, and up-to-date health clearance (TB, immunizations)


• Reliable transportation with valid auto insurance and ability to travel between patient homes

Home Health Physical Therapist Contract Details

• $2,436 - $2,663 per week*


• 8-week contract


• Paid by the hour - not by the visit


• 5–6 visits per day (45–90 minutes each, based on complexity)


• Travel time factored into productivity expectations
