Supplemental Health Care is seeking a Travel Home Health Physical Therapist to provide compassionate, patient-centered care in Upper Marlboro, Maryland and surrounding areas. This 13-week contract includes 40 hours per week and daily travel between patient homes.

Qualified candidates hold an active PT license in the assignment state, current BLS, and 1+ year of home health experience, with proficiency in EMRs and adherence to Medicare/OASIS-E guidelines.

Qualifikationen

• Active PT license in the state of assignment.
• Current BLS certification (AHA).
• 1+ year of recent home health experience.
• Experience conducting PT evaluations and developing therapy plans.
• Familiar with Medicare-certified home health guidelines and OASIS-E documentation.
• Experience with EMRs such as Homecare Homebase, NetSmart, or Epic.
• Clear background check, driving record, and health clearance (TB, immunizations).
• Reliable transportation and ability to travel between patient homes.

Aufgaben

• Perform initial and ongoing PT assessments and develop individualized plans of care.
• Provide skilled interventions for strengthening, endurance, pain management, and fall prevention.
• Conduct gait and balance evaluations, home safety assessments, and mobility training.
• Educate patients and caregivers on exercise programs and safety strategies.
• Collaborate with RNs, OTs, SLPs, and physicians for coordinated care.
• Document all care in compliance with Medicare, OASIS-E, and agency standards.
