Qualifications

• Minimum 2 years of home health OT experience.
• BLS certification required.
• Washington state license required if applicable.
• Experience with OASIS documentation.
• Experience with home exercise programs and patient/family education.
• Two professional references from the past 2 years required, at least one from a supervisor.

Responsibilities

• Evaluate and treat patients in home health OT.
• Develop and manage plans of care.
• Collaborate with patient care team to optimize outcomes.
• May oversee therapy assistants and float within the health system.
