Position Overview:

We are seeking a compassionate and skilled Registered Nurse (RN) to deliver high-quality, patient-centered care to individuals of all ages in their homes. The RN will collaborate with an interdisciplinary team to assess, plan, implement, and evaluate care, ensuring continuity and compliance with healthcare standards.

Key Responsibilities:

Conduct thorough evaluations of patients' physical, psychosocial, and environmental needs.

Utilize tools like the Outcome and Assessment Information Set (OASIS) for accurate documentation and care planning.

Care Planning and Coordination:

Develop, implement, and regularly update individualized Plans of Care (POC) in collaboration with patients, families, and the healthcare team.

Ensure plans reflect cultural, spiritual, and personal preferences.

Skilled Nursing Interventions:

Administer prescribed medications, manage IV therapies, perform wound care, and other specialized nursing tasks.

Monitor patient responses and adjust interventions as necessary.

Patient and Family Education:

Provide instruction on disease management, medication adherence, home safety, and self-care techniques.

Offer emotional support and counseling to patients and families, addressing end-of-life concerns when appropriate.

Supervision and Delegation:

Oversee the work of Home Health Aides (HHA), Licensed Practical Nurses (LPN), and other support staff.

Ensure tasks are delegated appropriately, considering the complexity of care and staff competencies.

Documentation and Compliance:

Maintain accurate and timely clinical records, including assessments, interventions, and outcomes.

Ensure compliance with regulatory standards and third-party payer requirements.

Interdisciplinary Collaboration:

Participate in team meetings and case conferences to discuss patient progress and adjust care plans.

Serve as a liaison between patients, families, and healthcare providers.

On-Call and Emergency Response:

Provide on-call nursing services during evenings, weekends, and holidays as scheduled.

Respond promptly to urgent patient needs and coordinate necessary interventions.

Required Qualifications:

Education: Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program; Bachelor of Science in Nursing (BSN) preferred.

Licensure: Current, unrestricted RN license in the state of practice.

Experience: Minimum of 1-2 years as an RN in a clinical care setting; at least 2 years of home health experience preferred.

Certifications: Current CPR certification; CHPN (Certified Hospice and Palliative Nurse) certification preferred.

Skills: Proficiency in Homecare Homebase (HCHB) software; experience with OASIS assessments; strong organizational and communication skills.

Other: Reliable transportation with valid driver's license and insurance; ability to work flexible hours and travel locally.
