Job summary

Franciscan Health Dyer Campus in Dyer, IN is seeking a Home Health Aide to support patients in coming home health settings. You will assist with activities of daily living, document care, and monitor vital signs as directed by the supervising clinician.

The role requires an active CNA or HHA license in Indiana, CPR certification, a valid driver’s license, and ability to travel up to 75% as needed. Prior home health or long-term care experience is preferred.

Qualifications

• High School Diploma/GED - Required
• Certified Nursing Assistant (CNA) OR Home Health Aide (HHA- State of Indiana Licensing Boart- Required
• Cardiopulmonary Resuscitation (CPR)- American Heart Association - Upon Hire- Required
• Auto Insurance - Upon Hire- Required
• Driver’s License - Upon Hire- Required
• 1 year Home Healthcare Experience - Preferred
• 1 year Long Term Care Setting Experience - Preferred

Responsibilities

• Assist patients with care and activities of daily living, as directed in the established Plan of Care
• Complete necessary documentation of care provided.
• Records intake, output, and vital signs as directed and communicates care, as appropriate.
• Provide assistance with personal hygiene and elimination needs, according to the established plan of care.
• Assist patients with ambulation and transfer using proper body mechanics.
• Communicate any change in condition or lack of patient progress.
• Schedule visits with the patient or caregiver.
