HealthForce Healthcare in San Leandro, California, seeks a Certified Occupational Therapy Assistant for a travel assignment in a skilled nursing facility. The 13-week position offers guaranteed 35 hours per week with a shift from 08:00 to 16:30, five days a week.

The role provides weekly pay ranging from $1,602 to $1,719, with fast credentialing and potential extension based on assignment needs.

Qualifications

• Strong communication, teamwork, and documentation skills are required.
• Active California COTA license is required or eligible.
• Ability to work in a skilled nursing facility environment.
• Prior experience in occupational therapy assistance in a rehab setting preferred.
• Availability to work 08:00–16:30, Monday through Friday (five days per week).

Responsabilités

• Provide occupational therapy services to residents focusing on ADLs, upper-extremity function, cognition, and safety.
• Evaluate residents and develop and monitor individualized treatment plans.
• Deliver evidence-based interventions and adaptive equipment training to promote independence.
• Collaborate with the OT, nursing staff, and interdisciplinary team to coordinate care and discharge planning.
• Document evaluations, treatment plans, progress notes, and discharge summaries timely and accurately.
• Ensure patient safety, infection control, and proper use of assistive devices during sessions.
