• Collaborate with the OT, nursing staff, and interdisciplinary team to coordinate care and discharge planning.
• Document evaluations, treatment plans, progress notes, and discharge summaries timely and accurately.
• Ensure patient safety, infection control, and proper use of assistive devices during sessions.

Responsibilities

• Provide occupational therapy services to residents under supervision, focusing on activities of daily living, upper-extremity function, cognition, and safety
• Evaluate residents, develop and implement individualized treatment plans, and monitor progress toward functional goals
• Deliver evidence-based interventions and adaptive equipment training to promote independence and safe functional performance
• Collaborate with the occupational therapist, nursing staff, therapy teammates, and the interdisciplinary team to coordinate care and discharge planning
• Document evaluations, treatment plans, progress notes, and discharge summaries in the resident record; ensure timely, accurate, and compliant documentation
• Ensure patient safety, infection control, proper use of assistive devices, and adherence to facility policies during treatment sessions

Requirements

• Active California COTA license (or eligibility per state licensing requirements as applicable)
• Ability to work in a skilled nursing facility environment and meet the demands of a geriatric rehabilitation caseload
