Connected Health Care is seeking a motivated and experienced Local Travel Physical Therapist for a 13-week contract assignment in Kansas City, Missouri. Job Description :

Evaluate, plan, and implement individualized treatment programs for patients with mobility, strength, or functional limitations

Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care

Monitor patient progress and adjust treatment plans as necessary

Educate patients and families on exercises and therapeutic techniques for continued care

Maintain accurate documentation of evaluations, treatments, and outcomes

Ensure compliance with facility policies and state / federal regulations

Education & Certification Requirements :

Graduate of an accredited Physical Therapy program

Active Missouri Physical Therapist license (or eligibility)

Current BLS certification (AHA preferred)

At least 1 year of clinical PT experience preferred
