Qualifications

Responsibilities

• Assist healthcare professionals in delivering patient care.
• Monitor and communicate patient condition to care team, physician, patient and family.
• Coordinate patient care across disciplines, including transfers and discharge as needed.
• Provide patient and family education appropriate to age and condition.
• Advocate for the patient’s physical and emotional well-being.
