Infinite Medical P.C. seeks a friendly Travel Nurse Practitioner or Physician Assistant with Cardiology experience to provide on-site care to residents in long-term care facilities.

This full-time role focuses on compassionate patient relationships and collaborative care within a dedicated team. Travel to multiple nursing homes in Connecticut is required, with a flexible schedule and opportunities to build meaningful, long-term patient connections while supporting heart-health programs.

Qualifications

• Nurse Practitioner license required.
• DEA license required.
• Minimum 6 months of experience in long-term care.
• Reliable transportation required.
• Willing to travel to nursing homes across Connecticut (100% travel).
• Must be able to lift up to 20 pounds and stand/walk for up to 6–8 hours per day.

Responsibilities

• Perform comprehensive patient assessments, including physical examinations and histories.
• Collaborate with cardiologists to develop and implement treatment plans for cardiovascular conditions.
• Administer medications, monitor responses, and adjust treatment plans as necessary.
• Conduct cardiac stress tests and EKGs, and other diagnostic procedures.
• Educate patients and families about cardiovascular health, preventive measures, and medication adherence.
• Travel regularly to multiple nursing homes within the region to deliver on-site care.
• Maintain a flexible schedule to accommodate facility visits and optimize patient coverage.

• Conduct cardiac stress tests, electrocardiograms (EKGs), and other diagnostic procedures
• Educate patients and their families about cardiovascular health, preventive measures, and the importance of medication adherence
• Travel regularly to multiple nursing homes within the designated region to deliver on-site care
• Maintain a flexible schedule to accommodate facility visits and optimize patient coverage

Requirements

• Nurse Practitioner License
• DEA License
• 6 months of experience in long-term care
• Reliable transportation
• Must be willing to travel to different nursing homes within the state of Connecticut (100% commitment)
• Must be able to lift a minimum of 20 pounds and stand / walk / work on your feet up to 6-8 hours per day
